Introduction

Thank you for participating in this valuable tool for your church.  The tabulated results of this survey will help Grace in its strategic planning process by identifying your church’s strengths, as well as opportunities for improvement.  This survey was developed by TAG, a research and consulting firm located near Washington, D.C.  The questionnaire that you will complete is built around 12 key factors.  While there are many questions that could be asked, these 12 factors represent the most significant elements of overall church health.  Your honest responses are very important in this process.  Results will be tabulated by TAG and individual answers to questions will be confidential. Your church will receive collective results only.

Taking the Survey

Do not labor over your decisions – your first instinct is usually right.  The survey will take you about 20-30 minutes to complete.  You will need to complete the survey in one sitting.  Your responses will be completely confidential.  Each member of a family over the age of 15 should participate individually. 

Answer each question according to the instructions.  When you have finished each set of questions (usually 10 questions), click “Next” at the bottom of the page.  You may change your answers at any point while you are logged on.  Once you log off, or click finish, your answers will be automatically processed.

Troubleshooting

• I can’t log on. 
• Make sure that you have typed the log on ID and password exactly as it appears.  Otherwise, you may experience a routine maintenance where the system is down for an hour or two.  Try again later.
• I can’t get past page one – it keeps taking me back to the beginning. 
• This is a problem with the way that your computer’s security features / network firewalls are configured.  This problem means that the your computer is not allowed to transfer the data to our network’s database (via the website).  This is due to a security feature within your own computing environment.  If you have an out of date Internet browser, you may not be able to participate from your computer.  Often, this issue is resolved by going into your Internet Browser, select Tools / Internet Options / Security – and then reset your security feature to “minimum” (or allow cookies).  TAG cannot provide any additional assistance with this.  You will need to contact your own network administrator or technology provider to help you remove the security features that are preventing data transfer.  If the church has made a computer available at the church office, you may want to use that computer to complete the survey.
